RURAL & COMMUNITY ACCESS

A local starting point for connected care.

Explore a community pilot designed around local access needs and the providers available to serve the area.

Planned use cases · initial market focus: Oklahoma, Texas & Florida

WHAT A PILOT COULD SUPPORT

01

An accessible community location

Evaluate a location people already know and can reach, such as a participating community facility or clinic.

02

A connection to a wider care network

Explore measurement collection and optional teleconsultation with the patient’s chosen provider, depending on local availability and supported connections to existing care systems.

03

Support beyond the visit

Define how people receive their summary, reach the care team, and arrange further care when appropriate.

SHAPING THE PILOT

Start with your setting.

The first conversation would establish:

  • Population served, geography, and local access priorities
  • Potential host sites, connectivity, and assisted-use needs
  • Local providers, referral options, and ongoing operating support

Define success together.

Agree measures such as reach, completed visits, reliable connectivity, and successful connections to follow-up care.

Plan the responsibilities.

A pilot would complement the local care network. It would need a clear service scope and referral process; the website and sample visit are not emergency care services.

Define the operating model together.

Clinical services, onsite support, integration, maintenance and follow-up responsibilities would be agreed before launch. Customer-operated, co-managed and managed-service options are under evaluation, not confirmed offers.
