WHAT A PILOT COULD SUPPORT

01

A place residents can reach

Consider a shared community location for guided measurements and access to a participating care program.

02

A bridge to local care

Support residents’ choice of provider or telehealth provider, including their existing practice where a connection is supported, with clear information about availability and next steps.

03

A manageable host experience

Define access hours, resident communications, cleaning, and support before installation.

SHAPING THE PILOT

Start with your setting.

The first conversation would establish:

  • Community size, resident population, and accessibility needs
  • Private space, connectivity, access hours, and onsite support
  • Clinical partners, service scope, and proposed payment arrangements

Define success together.

Agree measures such as resident participation, satisfaction, service availability, and connection to follow-up.

Plan the responsibilities.

Property managers would not receive individual health information simply by hosting a unit. Resident privacy and clinical data access would be defined for the program.

Define the operating model together.

Clinical services, onsite support, integration, maintenance and follow-up responsibilities would be agreed before launch. Customer-operated, co-managed and managed-service options are under evaluation, not confirmed offers.
